How To Take Azithromycin
Use Azithromycin exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended.
Follow all directions on your prescription label and read all medication guides or instruction sheets. Use the medicine exactly as directed.
Azithromycin oral is taken by mouth. Azithromycin injection is given as an infusion into a vein, usually for 2 days before you switch to azithromycin oral. A healthcare provider will give you this injection.
You may take azithromycin oral with or without food.
Shake the oral suspension before you measure a dose. Use the dosing syringe provided, or use a medicine dose-measuring device .
Use this medicine for the full prescribed length of time, even if your symptoms quickly improve. Skipping doses can increase your risk of infection that is resistant to medication. Azithromycin will not treat a viral infection such as the flu or a common cold.
Store at room temperature away from moisture and heat. Throw away any unused liquid medicine after 10 days.
Take the medicine as soon as you can, but skip the missed dose if it is almost time for your next dose. Do not take two doses at one time.

How Is A Bladder Infection Diagnosed
Bladder infections are usually diagnosed with a urine test.
- Urinalysis, which checks for white blood cells in the urine that can be a sign of infection
- Urine culture, which uses a sample of urine to try and grow bacteria in a laboratory to identify the type of bacteria causing the UTI and determine which antibiotics would be effective against that bacteria

Can Uti Symptoms Linger After I Take Antibiotics
Since UTI symptoms usually improve just a few days after starting antibiotics, youll want to talk to your healthcare provider if you notice that UTI symptoms are still hanging around after finishing your antibiotics.
Theres no need to panic, but you and your healthcare provider will want to make sure the antibiotics actually worked against your UTI. To do this, they may take another sample of your urine to see if the bacteria are still there or not. If the infection is cured, youll want to be sure there isnt a different issue thats causing similar symptoms.

Recurrent Urinary Tract Infections
Patients with three or more infections per year should be offered either continuous low-dose antibiotic prophylaxis, patient-initiated, or postcoital prophylaxis if the onset of infection is linked to sexual intercourse .7 Before a prophylactic regimen is chosen, a urine culture should be performed to determine the susceptibility of the pathogen. The duration of continuous prophylactic therapy is usually 6 months to a year. Unfortunately, within 6 months of discontinuing antibiotic prophylaxis, 40% to 60% of women develop a urinary tract infection, and prophylaxis must be resumed.20 Patient-initiated therapy at the onset of symptoms has been shown to be effective in young, healthy nonpregnant women.21 Short-course regimens have been advocated for patient-initiated therapy in compliant women with frequently recurring and symptomatic urinary tract infections. The major advantages of short-course therapy over continuous therapy are convenience and the avoidance of antibiotic toxicity symptomatic infections are not prevented, however. For postcoital prophylaxis, nitrofurantoin, trimethoprim-sulfamethoxazole, or fluoroquinolones taken within 2 hours after sexual intercourse have been shown to significantly reduce the incidence of recurrent cystitis.22, 23

Urine Drug Concentration And Clinical Efficacy
Antimicrobial drugs must achieve an adequate urine concentration, which must be maintained for a sufficient time for a drug to be effective in treating UTI.16 It has been suggested that clinical efficacy is observed when the urine drug concentration is maintained at a concentration 4-fold higher than the isolates MIC throughout the time between doses.9
Experimental studies in rats have shown that the time for which the plasma drug concentration exceeds the isolates MIC correlates to the magnitude of bacterial colony count reduction the longer the time for which the drug concentration remained above the MIC, the lower the urine colony counts.12 Successful eradication of bacteria within the renal parenchyma or urinary bladder wall is correlated to the plasma, not urine, drug concentration.
When prescribing time-dependent antibiotics, shortening the interval between drug administration is the most effective method to allow the tissue/urine drug concentration to exceed the MIC for the majority of the dosing interval.
- Drug elimination follows first-order kinetics, where 50% of the drug is lost in 1 half-life.
- In contrast, doubling the dose would only add 1 half-life to the dosing interval.
- To add 2 half-lives to the dosing interval, the initial dose would have to be increased 4-fold. The peak serum drug concentration achieved by this approach may exceed the window of safety, producing adverse drug effects.
The Pathogen: The Commensal Flora
UTI is frequently caused by organisms which are normal commensals in the distal urethra and adjacent sites. The most common route of infection is by ascension. The well-recognized gender difference in the prevalence of UTI is clearly related to the short length of the female urethra. Uropathogens are part of the normal faecal flora. These bacteria colonize the perianal region and then ascend in females to the introitus vaginae which is a reservoir for several uropathogens, particularly if the vaginal flora is not intact. Colonization spreads to the periurethral area, urethra, and bladder, strongly depending on sexual activity. Even during voiding, however, there is sufficient turbulence in the female urethra to allow backflow of organisms into the bladder. The role of normal vaginal flora in the defence against genital colonization with potentially pathogenic adhering E. coli has been demonstrated in several studies. The reported vaginal colonization rate of E. coli varies from 6 to 26% .

What Does It Mean If You Have Recurring Utis
Recurrent urinary tract infections are widespread. One study found that 44% of women who experience acute uncomplicated cystitis will have a recurrence later that year, usually within three months from the initial episode. Most providers define recurrent UTIs as two or more infections in six months or three episodes or more over a year .
Recurrent UTIs usually do not indicate a failure of the first treatment, though the same strain of germs may have reinfected you. If your last infection happened recently, your healthcare provider might ask for a urine culture to see if the drug was active. A UTI is considered a relapse of the same infection if the recurrence happens within two weeks of a previous episode .

Clindamycin is a powerful antibiotic usually reserved for very serious bacterial infections of the lungs, skin, blood, female reproductive organs and other internal organs. It carries an FDA black-box warning to alert doctors and patients about possible adverse effects, including swelling of the face and tongue and serious skin reactions.
Clindamycin may be given to patients who cant take penicillin because theyve had an allergic reaction to it. This antibiotic wouldnt be a first choice for a urinary tract infection, but you dont say whether your wifes infection failed to respond to other treatment before the Clindamycin was prescribed. One reason that Clindamycin should be used with care is that it disrupts normal flora in the intestinal tract, sometimes causing a severe intestinal condition called pseudomembranous colitis. This can occur during treatment or up to two months later. The symptoms to watch out for are abdominal pain, nausea, vomiting, and diarrhea. If any occur during or following treatment, be sure to contact your physician immediately.
