Overall, 7080% of patients treated with fesoterodine experienced a 50% improvement in UUI at Week 12 and more than 50% of patients became dry with fesoterodine 8 mg. Some patients achieved 100% resolution in urgency episodes and more than half of fesoterodinetreated patients normalised their daytime frequency at Week 12.
With respect to AEs, Wagg et al analysed data from 6689 patients included in fesoterodine studies and found that resolution of urinary urgency by either 50% or 100% at Week 12 was associated with a reduction in the number of reported treatmentemergent AEs. This unexpected finding was observed for discontinuation rate, dry mouth, constipation, central nervous system or cognitive adverse effects. Although not explained by the method it may be that patients may be less likely to express and to report AEs when the degree of benefit from treatment is either higher or reaches their expectations.

While this class of medications has long been known to be linked to memory troubles, it was not known whether the effects were permanent, says Michael Hochman, M.D., an associate professor of clinical medicine at the Keck School of Medicine at the University of Southern California. But this study suggests that the effects may not entirely reverse after stopping the medication.
The research doesnt prove that the drugs cause dementiaonly that there is an association between the two. However, this study does raise concerns, and anticholinergic medications for incontinence have several other well-established side effects, Hochman says.

If symptoms of urinary incontinence are thought to be caused by an enlarged prostate, different medications can be offered to reduce these symptoms. The prostate gland wraps around the urethra , and if it is enlarged, it could squeeze the urinary passage tight, making the emptying of bladder more difficult and incomplete.

TUESDAY, Jan. 14, 2020 Learning how to control the urge to urinate may be all the therapy men need to treat an overactive bladder, a new study suggests.
A combination of drugs and behavioral therapy seems to work better than drugs alone, but behavioral therapy alone also worked better than drugs, the researchers found.
The trial of 204 men with overactive bladder suggests behavioral therapy may be a good way to start treatment, the study authors said.
The study provides good evidence that for the group of men with overactive bladder, symptoms without obstruction from an enlarged prostate can be successfully managed with behavioral therapy alone, said Dr. Manish Vira, who was not involved with the study, but reviewed the findings. Hes vice chairman of urologic research at Northwell Healths Arthur Smith Institute for Urology in Lake Success, N.Y.
Many medications typically prescribed for overactive bladder have significant side effects, especially in older men using multiple medications, Vira added.
For the trial, Kathryn Burgio, associate director of research at the Birmingham VA Medical Center in Alabama, and colleagues randomly assigned men to six weeks of behavioral therapy alone, drug therapy alone, or combined drug and behavioral therapy. After the initial six weeks, all of the men received combination therapy for another six weeks.
