An 89-years-old man presented to the emergency department with left groin swelling with years of evolution, irreducible in the past two days. He also referred hematuria for two weeks. The physical examination revealed a large mass in the left groin with tenderness on palpation and the abdomen soft and depressible.
Video 1: Due to persistent hematuria requiring transfusion support, an abdominal and pelvic computed tomography was performed showing a left groin hernia containing part of the bladder that had irregular parietal thickening with enhancement after intravenous contrast administration.
Outcome: The surgical management consists of removing the tumour with a safety margin and repairing the hernia. If there is muscle invasion and the patient is fit for surgery, radical cystectomy is the best option. If there is no invasion of the detrusor muscle, the submucosal cystectomy is curative.

Inguinal hernia repair is a regularly done surgical procedure, which may sometimes surprise the surgeon with its peculiar contents.
The urinary bladder can be contained in inguinoscrotal hernia and, more rarely, in perineal or obturator hernia. In 1951, Levin proposed the term of scrotal cystocele to indicate a large scrotal hernia of the urinary bladder .
Inguinoscrotal herniation of the bladder is a rare clinical entity, with a frequency between 0.5% and 4% of all inguinal hernias . It is more frequent among males, over 50 years old. The most important risk factors of scrotal bladder hernia include body mass index > 30, past injuries of the pelvis, previous abdominal wall or pelvic surgery, bladder obstruction , and bladder wall diseases . We report on a rare case of whole bladder and pelvic ureter herniation in a right inguinoscrotal hernia, preoperatively investigated with computed tomography scan and treated by prosthetic hernia repair.
The bladder can partially or entirely herniate into the inguinal canal when the whole bladder and ureters migrate into the scrotum, it may cause obstruction, calculi, vesicoureteral reflux, hydronephrosis, infection, and acute renal failure .

Herniation of the urinary bladder into the inguinal canal is rare and constitutes 13% of all inguinal hernias. Bladder herniation is usually asymptomatic and is determined incidentally however, it may rarely present with urinary obstruction symptoms. Here, we described an incidental bladder hernia in a 76-year-old man who is evaluated in radiology department for fever and left-sided flank pain for three days duration. He had no previous history of trauma, medication abuse, surgery or familial bladder disorders. His additional co morbidities included hypertension and diabetes mellitus. There were no remarkable findings on abdominal examination except left side loin tenderness. On evaluation Computed tomography revealed that a left inguinal hernia with fluid density lesion continuous with left lateral bladder wall. No bowel was contained in the hernia . The inguinal hernia was confirmed as bladder hernia by CT cystogram . Inguinal hernia usually contains the peritoneum that surrounds the abdominal organs. Bowel loops, omentum, or extraperitoneal fat can be detected within the hernia sac in the inguinal canal. However, as in this case, the bladder can also be present within the hernia sac. Early diagnosis with radiological imaging is important to prevent complications during surgery.

Herein we present an unusual case of urinary tract obstruction caused by herniation of the urinary bladder into the inguinal canal. While some herniation of the bladder into the inguinal canal can be seen in up to 4% of hernias , most reported cases occur in elderly men and are associated with greater or lesser degree of distal urinary tract obstruction, most often due to an enlarged prostate increased abdominal pressure due to bladder neck obstruction is a common reason for elderly patients to seek surgical consultation for a previously asymptomatic inguinal hernia.
