A study published online in The Lancet Oncology states that although there are no major safety concerns associated with ARBs, a previous trial had reported a significantly increased risk of fatal cancers in patients receiving the ARB candesartan compared with a placebo.

Target Blood Pressure Goal In Patients With Cancer

Patients with cancer have traditionally been excluded in the large-scale hypertension trials. Based on the best available evidence, the 2017 ACC/AHA Hypertension guidelines outline a series of recommendations that are generally applied to the care of the oncologic patient .

Patients who have an estimated 10-year ASCVD risk of 10%, or who have additional cardiovascular comorbidities such as type 2 diabetes mellitus or chronic kidney disease, should be treated to a goal BP< 130/80mmHg. Patients who have an estimated 10-year ASCVD risk of 10% with no additional cardiovascular comorbidities should be treated to a goal BP< 140/90mmHg, though it may be reasonable to treat these patients to a goal BP< 130/80mmHg as well . Figure illustrates a reasonable stepwise approach to identify and manage hypertension in the context of cancer treatment with the overarching goal of reducing the burden of CVD in this at-risk population. Randomized clinical trials with a focus on optimal BP goals and management in oncologic patients are needed to better inform contemporary clinical practice.

In late 2018, a few drug manufacturers reported that some batches of their blood pressure medications tested positive for contamination by a cancer-linked chemical called N-Nitrosodimethylamine and other related impurities. Consuming high doses of NDMA could potentially cause a person to develop cancer, but the amount of the chemical in the recalled blood pressure medications does not present an urgent threat to your health.

Out of an abundance of caution, the FDA initiated a recall of these specific medication batches. The recalls include a particular class of blood pressure medications called angiotensin II receptor blockers . These medicines work by reducing blood vessel constriction caused by the hormone angiotensin. Beginning in January 1992, all case patients and control subjects were asked to donate a blood sample. Seventy-four percent of the case patients and 79% of the control subjects donated blood. But this study suggests that the effects may not entirely reverse after stopping the medication.

The research doesnt prove that the drugs cause dementiaonly that there is an association between the two. However, this study does raise concerns, and anticholinergic medications for incontinence have several other well-established side effects, Hochman says.
