How Do Doctors Test For Bladder Infection

Finally A Practitioner Who Listened And Understood Embedded Uti

Easy@Home Urinary Tract Infection Tests

In my research I came across the words occult bacterial infection and this struck a chord with me. I was then extraordinarily lucky to come across a very knowledgeable and informative online group and learnt about the work of an English Professor who ran a specialist clinic in London treating complex, recalcitrant urinary infections.

I spent the next couple of years managing my symptoms the best I possibly could and learning everything I could get my hands on about embedded bladder infections.

In 2016 I finally travelled to London to see the Professor. At my appointment he asked for a fresh, undiluted urine sample which he looked at straight away under his microscope. He was looking for signs of infection, in particular white blood cells and epithelial cells.

He then listened closely to my UTI history and asked lots of questions about my symptoms. Unlike all the other doctors I had seen in Australia, he seemed very interested in my symptoms. He also performed a quick, non-invasive physical examination.

I was diagnosed with a chronic UTI and he explained Id probably had the infection for some years.

He started me on antibiotic treatment immediately and warned me that it was likely to take some time to completely clear this type of embedded infection.

The Situation In Australia Needs To Change

We desperately need a specialist in Australia who is educated and trained in diagnosing and treating chronic, embedded UTIs. At the moment, there is just one website, Chronic UTI Australia, that addresses this issue in the region.

This whole experience has been a horrible ordeal. In Australia I was put through various testing, including ultrasounds , CT scans, cystoscopy, several pap smears and a pelvic MRI.

I was angrily berated by a specialist until I cried at their desk. I was told by one doctor that I research too much, and told by another it was time to focus on my life and not my bladder.

Inadequate test results were given more credibility by doctors than the symptoms I was experiencing which is not only disheartening, but insulting.

Getting Tested For A Uti

Testing for a UTI usually takes place in a doctors office, laboratory, or hospital. Most tests for UTIs involve a urine sample obtained by clean-catch or catheterization. Your doctor can help determine which method is appropriate for you.

  • Clean catch urine samples are collected by patients with special precautions to prevent outside germs from getting into the urine sample. Patients are given instructions on obtaining a clean catch sample and avoiding contamination.
  • Catheterization involves inserting a thin rubber tube through the urethra into the bladder. When performing this procedure, the urine is collected in a sterile container for testing before removing the catheter.

Many people say that cranberry juice can help treat, or even prevent, a UTI. Researchers are currently looking into the topic, but havent found a definitive answer yet. Healthcare providers recommend drinking lots of fluids if you have, or have a history of getting, a UTI. Adding a glass of unsweetened cranberry juice to your diet isnt a proven way to prevent a UTI, but it typically wont hurt you either.

Urinary Tract Infection Testing With Your Healthcare Provider

Your healthcare provider will ask about symptoms and past medical history.

The most frequent test performed is a urinalysis. To prevent bacteria on your skin from contaminating your urine sample a clean catch urine sample may be recommended, Youll be sent to a bathroom with sterile wipes and a collection cup for your urine.

To provide a clean urine sample:

  • Women: Sit on the toilet, spread the labia, and wipe the labia area and the opening of the urethra with a sterile wipe front to back. Urinate briefly into the toilet with the labia spread. Then, stop the flow of urine, hold the cup in place, and urinate into the sterile cup provided.
  • Men: Clean the head of the penis with a sterile wipe and urinate into the toilet briefly. Stop urinating and then urinate into the sterile cup.

This urine sample is then tested for bacteria and white blood cells. A healthy person can have bacteria in their urine, so a diagnosis of a UTI is made based on the results of the urinalysis along with symptoms.

In cases where a UTI keeps coming back, is serious or is complicated, a urine culture may also be done. Although this test is not needed for diagnosis, it can help find the best treatment because it will tell your healthcare provider what type of bacteria are causing the infection. It usually takes about two days to get results of the culture.

Diagnostic Uti Test Tips

Leukocytes In Urine Std
  • If you have recurring UTIs, ask your doctor for a standing order for a UTI test at a laboratory near you. Then simply let your doctor know each time you drop off a sample.
  • You should use a cup provided by your physician to collect a sample and should store it in a fridge if its going to take you more than an hour to drop it off.
  • If youve had a lot of negative urine samples, ask if your urine can be cultured for more than the standard 48 hours. A little-known fact: you can ask to have your urine kept for up to seven days, which is more likely to show a rare UTI causing bacteria.
  • If youve had a lot of symptoms, ask your doctor to take seriously any bacteria that show up in your sample. When two or more bacteria are found, they are often labeled as contamination because the standard thinking is a single type of bacteria causes a UTI. However, if you have symptoms, several bacteria may be working together to cause your urine infection.
  • Some women get UTIs with less bacteria than doctors consider significant. Normally, UTI is diagnosed with at least 100, 000 colony forming units per milliliter of bacteria. However, if you have symptoms, fewer bacteria could be causing your symptoms . Ask your doctor if any bacteria grew, even if it is a small amount.

Studies Tie Human Bladder Infections To Antibiotics In Chicken

Many labs ignore those low-level tests, says Dr. Thomas Hooten, a professor at the University of Miami School of Medicine who led the study.

If doctors are going to do urine cultures they should at least insist that labs look for low levels of E. coli, Hooten says. He told Shots that he would say: “Look guys, low colony counts of E. coli are meaningful, don’t discard those results.”

The results were just published in the New England Journal of Medicine.

The study involved 226 women, aged 18 to 49, and got samples both from the midstream test and by putting a catheter in the bladder.

When comparing those samples, something strange showed up, surprising the doctors.

About one-quarter of the women had no bacteria at all in the bladder, even though they had classic symptoms of infection. And quite a few of those women did have lots of bacteria in their midstream test.

“We would have assumed that the reason you had high numbers in the midstream culture is because they had high numbers in the bladder,” says Donnenberg, who wrote an editorial accompanying the study.

Maybe those women have an infection of the urethra rather than the bladder, Donnenberg told Shots, or maybe there’s something else going on that the doctors weren’t aware of. The doctors really don’t know.

    The Role Of Uti Testing

UTI testing may be performed if a doctor suspects a UTI based on a patients symptoms and a physical examination. Laboratory tests can diagnose or rule out UTIs. UTI testing can also identify the bacteria or other microbes that are causing the infection, which helps doctors decide how to best treat the infection.

How To Get A Diagnostic Test For Uti

    Diagnosing and Treating Bladder Cancer

If you have UTI symptoms and must know for sure if it really is a UTI, choose a diagnostic test instead. The most widely available diagnostic test is a microscopic urinalysis and urine culture.

Generally, we look at your urine under a microscope . Simply adding the urinalysis increases the sensitivity of urine testing to 96% . Then, we place the urine into a petri dish with bacteria-friendly growth medium and wait for the bacteria to grow.

However, the standard urine culture test, even coupled with microscopy might not be enough. If your UTIs persist while your culture comes back negative, you might want to try a DNA-based urine test .

    Preventing Bladder Infections In Women

In women who experience three or more bladder infections in a year, these measures may help:

    • Increasing the intake of fluids

    • Urinating often

    • Urinating within a short time after sexual intercourse

    • Avoiding the use of spermicides

    • Taking antibiotics continually in low doses

    • Applying estrogen cream to the vulva or inserting estrogen suppositories into the vagina

Diagnostic Tests Confirm Diagnose In The Patients With Symptoms

A diagnostic test diagnoses people who are very likely to have the disease because they have common symptoms.

For example, a diagnostic test is perfect for a woman who has some classic symptoms of a UTI: goes to the bathroom 15 times a day, and has pain with urination. She could use an OTC UTI test strip, but the result will be less accurate.

OTC UTI test strip does not rule in disease. For that, you need a diagnostic test. Diagnostic tests are specific, if they are negative, you can be sure you dont have the disease.

So if you already have UTI-like symptoms, you want to rule all other reasons before starting a treatment for UTI. For example, if you think you have a UTI when in fact you have a kidney stone, you waste precious time and resources taking antibiotics or herbal remedies that will do nothing to get rid of the stone. Diagnostic tests shine at weeding out all the people who are likely to have a disease but dont.

You Can Visit An Urgent Care Center For Uti Treatment

Its likely you trust your OB/GYN or primary care physician to help determine the best course of treatment when ailments arise. But demands for medical care can make it difficult for you to get in to see you doctor, not to mention if you develop symptoms for a urinary tract infection at a time when your physicians office is closed.

Urgent care centers, like GoHealth Urgent Care, routinely treat UTIs. With extended hours and same-day walk-in appointments, its easy to get answers and relief from a healthcare professional. At a GoHealth Urgent Care center, well perform a urine test, also known as a urinalysis, to determine if you do indeed have a UTI. On-site laboratories allow us to review urine cultures and ensure we prescribe, if necessary, the best course and type of antibiotic treatment for you.

If seeing your OB/GYN or primary care physician is not an option, and youre wondering whether to go to an urgent care center or the emergency room, you should consider these differences. Emergency rooms are usually reserved for just that emergency or life-threatening situations, like strokes, head trauma or difficulty breathing. UTIs, while painful, are not life-threatening, especially if caught and treated early. Plus, visiting an urgent care center for UTIs typically costs far less than an emergency room, and may be covered by your health insurance.
